Our Services Include:
- Day Habilitation
Our Day Habilitation program provides structured supports to help individuals maximize inclusion in the community while maintaining their independence. Services are offered Monday through Friday, 8:00 a.m. to 4:00 p.m., in environments designed to promote personal growth and social engagement. Available for residents of Wicomico, Somerset, and Worcester Counties. - Personal Support
Personal Support services assist individuals living in their own or family home with developing skills to enhance independence. Our team provides guidance in daily living activities tailored to each individual’s unique needs and goals. We also offer specialized in home services in Edgewood, Maryland to ensure personalized support for residents in that area. - Community Development Services (CDS)
Community Development Services focus on building skills that increase independence and community involvement. Individuals are encouraged to participate in community events, volunteer opportunities, and travel training based on their interests, abilities, and personal goals. - Nursing Services
CHNS Nursing Services support positive health outcomes by promoting access to community resources and strengthening each individual’s existing support network. Our goal is to ensure comprehensive care that meets the unique needs of every person we serve. - Employment Services
Our Employment Services help individuals achieve meaningful and desired employment opportunities. Programs include Job Discovery, Job Development, Follow-Along Supports, Ongoing Job Supports, Co-Worker Employment Supports, Customized Self-Employment Supports, and Career Exploration. - Supported Living
Supported Living services provide individualized assistance to help individuals live independently in their own homes. Supports are tailored to each person’s abilities, preferences, and goals, fostering autonomy while ensuring necessary guidance and care. - Respite
Respite services offer temporary relief to caregivers by providing short-term, high-quality care for their loved ones. This service helps maintain balance in family routines while ensuring individuals continue to receive attentive support. - Transportation
